"I hereby make application for participation in the Legal Services Program as a member in good standing of CPOA. I understand that participation in the program is not automatic or guaranteed. My completed application will be reviewed and I will be notified within 90 days whether I have been accepted into
LSP. Legal Services will be provided to me during this interim period should I meet all other eligibility requirements. I further understand that any material misrepresentation on this application may invalidate any program coverage."
